What to write on a quilt label

Debra,
I have a top purchased in a thrift store. I
will probably make a label as follows:

Name of Quilt
Top Pieced by Anonymous Quiltmaker
Purchased in ........ Thrift Store
in Newport News, Virginia
Quilted and Finished by Pat.........
Town, State, Date

You could do something similar.
